Shoei RF1000 - which you can get on ebay for $175 from a guy - Look for my post about it on page two or three in General Topics here.
The guy who did it is @ Chaotic Customs in Tomball, TX. I love it. Always getting great comments on it too. Never seen one like it to date. Its nice to have a one of a kind. If you want their info - PM me. Or find a reputable custom paintshop in your area.... You'll be surprised what the artists who work there can do with an airbrush! Artwork ran me about $350. I forget exactly.... But if you figure that there are a million helmets out there that run 400-500 bucks and everyone has it... This ones just mine!
