rear wheel assembly

Took my rim to the shop today to get a tire put on. They also replaced the bearings on the rim, not the ones on the sprocket housing.  When I put the cush drive back together and tightened everything back up the nut went beyond the holes for the cotter key before tightening up. This isn't the first time I changed tires like this and the nut has never went this far in. I took the wheel back off and everything seems to be ok. Ifanyone else has ran into this any help would be appreciated. Thanks in advance.

You put the big spacer between the wheel and the cush drive sprocket, and the blocks that go against the adjusters on both sides of the wheel?
